Dolan Center Wins Redesignation

The Dolan Family Health Center is the first Northwell Health ambulatory site to achieve Patient Centered Medical Home designation by the National Committee for Quality Assurance under the latest New York State standards. 

 Northwell said Patient Centered Medical Homes (PCMH) foster relationships between medical practitioners and patients in a more comprehensive and cohesive way than can be achieved through individual health care appointments.

Patients are overseen by a clinician-led team that helps patients coordinate their care throughout the health system.

The Dolan Center received its first PCMH designation in 2014.

“This is a significant recognition that demonstrates the Dolan Family Health Center’s commitment to putting the patient at the center of everything that we do,” said Joseph Volavka, PT, senior administrative director of the Dolan Family Health Center. “I appreciate our staff’s hard work and dedication that made this re-designation possible.”

The Dolan Family Health Center provides a wide array of primary health care services for children and adults while providing high-quality care for the underinsured and uninsured.
